Better Health for the Delta
Description : "Better Health for the Delta" mobilizes Lincoln Parish residents in an effort to enhance health and quality of life. This project uses a needs assessment and public awareness campaign. Town leaders are recruited to participate in a comprehensive needs assessment project that identifies the determinant of health. From the results of the needs assessment, stakeholders select and prioritize programs that have proven to be successful. Goals include 1. Develop a local provider network that will coordinate a parish-wide planning process (i.e., service inventory provider network) 2. Identify the community needs, barriers, and facilitators of services 3. Assess current services regarding cultural sensitivity and relevance 4. Expand the awareness of the roles and benefits of local health networks, as well as develop and field test low cost preventative health care programs.
